Part-time contract: Up to March 31, 2027

Reporting to the Recreation Supervisor – Senior Services, this position leads the development, coordination, promotion, and evaluation of the Social Prescribing Program. The role connects older adults with community-based programs, services, and supports that enhance health, wellbeing, and belonging. Working closely with healthcare providers, community organizations, and social service agencies, the successful candidate strengthens referral pathways, supports participant engagement, and advances integrated approaches between healthcare and community recreation systems

Accountabilities:

Community Partnerships and Outreach

  • Develop and maintain partnerships with healthcare providers, community agencies, and social service organizations.
  • Conduct outreach, presentations, and networking activities to promote social prescribing services and increase community awareness.
  • Support referral partner communications and share program impact stories.
  • Participant Services
  • Coordinate and manage social prescribing referrals.
  • Assess participant needs, develop individualized connection plans, and facilitate access to appropriate community supports.
  • Provide follow-up support to encourage participation and positive outcomes.
  • Maintain current knowledge of community resources and services.

Program Development & Evaluation

  • Support the development, implementation, and continuous improvement of social prescribing initiatives.
  • Monitor participation, referral activity, and outcomes; prepare reports and evaluation summaries.
  • Recruit, train, and support volunteer ambassadors who enhance participant engagement and social connection.
  • Administration
  • Support grant administration, including outcome tracking, financial monitoring, data collection, and reporting requirements.
  • Maintain participant information in accordance with privacy legislation and organizational policies.
  • Develop program resources and communication materials.
  • Attend meetings, professional development opportunities, and perform other related duties as assigned.

Health and Safety

  • Comply with the Occupational Health and Safety Act and regulations, Workplace Safety and Insurance Act and Town Health and Safety policies and procedures in the carrying out of daily activities.
  • Prompt reporting of workplace accidents and incidents and unsafe conditions
  • Using or wearing the safety equipment, protective devices or clothing as required by the Town.


Qualifications:

  • Diploma or Degree in Recreation, Social Work, Health Promotion, Community Development, Gerontology, Public Health, or a related field.
  • Minimum two (2) years of experience in community development, health promotion, recreation programming, social prescribing, client services, or a related field.
  • Experience working with older adults and individuals facing barriers to participation.
  • Knowledge of community resources, healthcare systems, and social support services.
  • Demonstrated experience in participant assessment, referrals, outcome measurement, and reporting is considered an asset.
  • Experience with grant-funded projects or community health initiatives is considered an asset.
  • Excellent communication, facilitation, partnership-building, and stakeholder engagement skills.
  • ·Ability to work independently and collaboratively within interdisciplinary teams.
  • Current Intermediate First Aid/CPR certification.
  • Valid Ontario Driver’s License and access to reliable transportation

Position Details:

This posting is for an existing vacancy. This position is available on a part-time basis up to March 31, 2027.

Compensation:

The hourly rate for this temporary part-time position is $22.97–$27.35 up to a maximum of 25 hours per week. Hours will vary and may require flexibility in scheduling, including some evening and community-based commitments.

Location:

This position will be a combination of office and community-based work with the occasional work from home opportunity.
