ActiVan Eligibility
Eligibility
Persons interested in using ActiVan must complete an application form.
If registering as a SENIOR, applicants must provide proof. (a photocopy of birth certificate, passport, etc.) that they qualify by virtue of age – 65 years or older.
If registering as a person with disability, applicants must have a doctor or medical practitioner answer the health related questions on the application form.
In both cases, the information provided will remain strictly confidential.
